Connect the Patient Hub to your Teams Hub

Your Teams Hub and Patient Hub are automatically linked to work together behind the scenes. This integration allows you to sync actions and rewards across both platforms—saving time and increasing engagement.

What it does:
When a key event happens in Patient Hub—like reaching your goal for patient reviews or completing a referral milestone—the system can automatically trigger rewards or recognition for your team in the Teams Hub.

Why it matters:
This ensures that everyone stays motivated and aligned around shared goals without the need for manual tracking or follow-up. It connects team effort with real-time outcomes, making it easier to celebrate wins, encourage consistency, and drive results across your practice.

Let's get started!

Step 1: Integrations 

Navigate to Settings⚙️>Integrations. Click on Patient Rewards Hub or Culturello Patients.

No tech headaches here—your accounts are already connected by default. But if you need to manage access, you can always click Account Admin to disable or reactivate the connection.

Step 2: Aligning Rewards to Actions

Go to Settings ⚙️ > Integrations > Patient Rewards Hub > Reward.

You’ll land on your Automations screen—where all your reward triggers live. This is your command center to:

  • View current reward automations
  • Edit past integrations
  • Create new, outcome-driven triggers

To create a new automation, click Add. Then choose from pre-built reward templates to get started fast.

Step 3: Customize the Journey

Pick a template and make it your own:

  • Title your automation
  • Define the Who (who receives points),
  • Set the What (what behavior earns it),
  • Clarify the When (when it triggers)

You’ll see default point values and messages built in—but these are fully customizable.

📣 Want to make it more you? Click Communication to tailor the email and in-app message that members receive. Visuals and copy are yours to adjust.

🎉 Communication happens automatically—once the trigger fires, your members will be celebrated without you lifting a finger.

Final Step: Activate & Amplify

Once you're happy with your automation, just click Activate.

You’ll be returned to your main automation dashboard where you can view, adjust, or build more.
