Preparing your patient data for import into your Hub is one of the final steps when launching your new membership program.
Step 1: Export Your Patient Data From Your Practice Management System (PMS)
To segment your current members by their status, please send us separate files for each group (New Patient, Observation, Active Patient, Retention) so we can upload and tag them accordingly in the Hub.
File Format Guidelines:
- Data should be formatted as .csv, .txt, or .xls files.
- Data should be separated into fields using columns or commas.
Fields for Import:
Below are the fields we can import into the Hub. If a field is not applicable, leave it blank, but still include it in the file. Required fields are marked with an asterisk (*)
- First_name*
- Middle_name
- Last_name*
- Address1
- Address2
- City
- State
- Zip
- Country
- Home_phone
- Mobile_phone
- Responsible_email
- Responsible_cellphone
- Responsible_homephone
- Birth_month*
- Birth_day*
- Birth_year*
- Patient_status
An Excel file formatted with the information needed is attached to the end of this article. ⬇️ If you’re unsure how to export your data, your practice management software vendor can provide assistance.
Step 2: Transfer Patient Data to Your Hub
To ensure that your data remains private, please upload the files directly into your Hub by following these steps:
- Log into your account at my.patientrewardshub.com. This step must be performed by one of the admins on the account.
- In the same web window, remove the current URL and type: https://my.patientrewardshub.com/manage/uploadpatientdata (do not copy and paste the URL, as it will not work if pasted).
- Name each file according to this format [yourpracticename_PATIENTSTATUS_PRHreport]
- Attach and upload the file.
Note: Patient data may only be transferred to the Hub in this manner if your Hub is not yet live. Please reach out to our support team for an alternative way to transfer data if your Hub is live.
